Ongoing Care Coordination

  • On a flat fee basis for a period not to exceed 12 months, establish a service plan to provide the appropriate selections from the services listed below to clients and/or their survivors who had engaged the firm for some or all of the services described above:
    • Continue to serve as the client’s authorized representative to complete and file annual ALTCS review forms, and communicating with the agency
    • Research and communication as needed to maintain ALTCS status
    • Advocacy for health care issues for the Medicaid and/or Aid and Attendance client.
    • One conference with the Medicaid beneficiary’s survivors to discuss any estate recovery issues. (A separate fee agreement will be required to represent the client in the Estate Recovery Process)
    • One conference with the VA Aid and Attendance beneficiary’s surviving spouse to discuss the survivor’s eligibility for benefits in his or her own name. (A separate fee agreement will be required to represent the client in the application process for those benefits.)
